Medical SEO services for healthcare groups with 5 to 150 locations

Citemill runs medical SEO services for dental, dermatology, urgent care, optometry, physical therapy and other healthcare groups with 5 to 150 locations. We fix each location's Business Profile, provider pages, structured data and review flow so more locations appear in Google's map pack and AI answers, then count how often ChatGPT, Perplexity, Gemini and AI Overviews name each one. Programs start at $1,900 a month plus $290 per location.

What our medical SEO services include

Every location gets accurate listings, a location page with its providers, a review flow that stays inside HIPAA and FTC rules, healthcare structured data and a monthly count of which AI engines name it. Health is a "Your Money or Your Life" topic, and Google's rater guidelines say "Trust is the most important member of the E-E-A-T family", so every deliverable is built to be checked.

  • Location and provider pages with clinical review. Each location page lists its services, providers, accepted insurance and hours. Provider pages show real credentials, and a named clinician reviews clinical copy. Google's helpful content guidance encourages accurate bylines. Where boards regulate claims, such as Texas requiring "General Dentist" in specialty ads or California limiting "board certified" to qualifying boards, we write to the rule.
  • Practitioner and department listings. Google's guidelines allow a practitioner profile only for public-facing providers reachable at the location during its hours, and a department profile only for a distinct unit. We clean up duplicates, and every location links to its own page, as Google's business links policy requires for multi-location brands.
  • A review flow within HIPAA and FTC rules. Every patient gets the same request. No incentive depends on sentiment, which the FTC's review rule bans with penalties up to $53,088 per violation. Replies never confirm someone is a patient: HHS settled with New Vision Dental for $23,000 in 2022 and Manasa Health Center for $30,000 in 2023 over replies that disclosed patient information.
  • Tracking that respects HHS guidance. A June 2024 court order vacated only part of the HHS bulletin on tracking technologies. HHS still says a page that lets people book appointments without logging in "may have access to PHI". We keep third-party pixels off booking and symptom pages and flag the rest for your counsel.
  • Healthcare structured data. MedicalClinic, Dentist, Optician or Physician markup with address, hours, geo and departments, and IndividualPhysician pages linked to the clinic through practicesAt. Google asks for the most specific business subtype and says AI features need no special schema, so we use markup for accuracy.
  • AI answer tracking per location and specialty. A fixed set of patient questions per city and specialty, such as "urgent care near me", run on ChatGPT, Perplexity, Google AI Overviews and Gemini on Essential, plus Claude and Copilot on Complete.

How our medical SEO agency works with your locations

We start with a free check, then a 10-day baseline, then fixes, fastest wins first. Your team spends about 2 hours on onboarding and 2 to 3 hours a month after that, plus clinical review time for new pages; a developer needs 6 to 10 hours once. That is how a search engine optimization agency for medical groups should run: baseline first, fixes second.

  1. Free check 3 locations on one AI engine, reply in one business day
  2. Audit, 10 days every location on every engine, specialties agreed in writing
  3. Weeks 3 to 6 listings, practitioner cleanup, markup, tracking review
  4. Weeks 5 to 12 provider pages through clinical review, reviews, media, monthly report

What it costs per location

Pricing is a published formula: a monthly network fee plus a per-location rate that drops as the network grows. Month to month, no lock-in, and deliverables stay with you.

LocationsEssential, monthlyComplete, monthlyAudit, one-time
Network fee$1,900$2,900$2,900
Per location$290 (1 to 10), $232 from 11$460 (1 to 10), $368 (11 to 25), $340 (26+)$150 per location above 10
10 locations$4,800$7,500$2,900
25 locations$7,700$12,100$5,150
50 locations$13,500$19,900$8,900
150 locations$36,700$53,900$23,900

The audit fee is credited in full to your first month. Complete adds Claude and Copilot, neighborhood content, local media placements, a review pipeline and an experiment log.

Who our medical SEO specialists work with, and who they don't

This fits healthcare groups with 5 to 150 locations that can give us access to their Business Profiles and location pages and can name a clinician to review clinical copy. Consolidation is real: 16.1% of US dentists were affiliated with a DSO in 2024 (ADA, August 2025), and nearly 58% of new urgent care centers in 2025 opened under operators with 10 or more sites.

It fits:

  • Dental and orthodontic groups, dermatology, urgent care, optometry, physical therapy and chiropractic, hearing clinics and med spas.
  • PE-backed platforms adding locations every quarter, and franchisors who need one listing standard; our franchise SEO service covers the governance side.
  • Groups in the US first, and also in the UK, Canada, Australia, Ireland, New Zealand and Singapore, where we check local advertising rules first.
  • Marketing teams that want organic and AI visibility measured per location, not as one brand average.

It doesn't fit:

  • Hospitals and health systems. Their department and roster structure is a different job.
  • Single-location practices. A local specialist will serve you better for less.
  • Groups that cannot publish provider credentials or get clinical sign-off on copy.
  • Anyone who wants a guaranteed ranking position or review tactics that filter out unhappy patients.

How to choose a healthcare SEO agency, firm or consultant

A healthcare SEO consultant audits and advises, while a healthcare SEO company or a healthcare SEO firm also does the work on listings and pages, clinic by clinic. When you compare healthcare SEO services, ask four things.

Does the SEO agency for healthcare report rankings and AI answers for every clinic? Does its tracking keep protected health information out? Does a clinician review medical claims? Who owns the work if you leave? The same applies to an SEO agency for doctors and to clinic SEO services sold as a package.

What we measure per location and when results land

We report per location, per engine and per specialty query, and we expect listing fixes to show within 3 to 6 weeks and AI answer share to move over 3 to 4 months. Both are estimates, not promises. We track chat engines closely for location queries because Javvo found AI Overviews on under 2% of general provider searches, against 99% of general health questions. Ask any medical SEO experts you talk to for the same per-location view.

Each month, every location gets these numbers:

  • Answer share. In how many of its fixed queries each engine names the location. Answers vary between runs, so we report counts, not screenshots.
  • 3-pack presence. Whether the location appears in Google's local pack for its core specialty queries.
  • Review position. Total reviews, average rating, reviews from the last 90 days and the gap to the brand median. In Tebra's 2025 survey of 3,964 US adults, 53% would not consider a provider with fewer than 4 stars.
  • Listing health. Whether each profile links to its own page, has the right category, has no practitioner duplicates and passes structured data checks.
  • Actions. Calls, website clicks, direction requests and booking clicks from each profile. Zocdoc reports 43% of its bookings happen outside office hours, so a working online booking link counts.

We also mark every Google core update on the report. Google ran five between March 2025 and May 2026, and after the December 2025 update Amsive measured drops from 19% at WebMD to 46% at Verywell Health. A dip in that window means something different from a dip after our changes.

AI is still a minority channel for finding a doctor. We track AI answers because AI use for local recommendations rose from 6% to 45% in a year, not because it replaces the map pack. Rankings alone mislead too: on Near Media in September 2026, Andrew Shotland described a 100-location franchise whose rankings rose 20% to 30% while calls fell 40%.

What our study of 481 healthcare chains found

ChatGPT named the healthcare chain in 15 of 95 local patient questions we tested, and more than a third of chains with location pages send at least one listing to the homepage. The data comes from 481 healthcare chains with 5 to 150 locations, part of our study of 562 multi-location chains in seven countries, collected from Google Places and each brand's website in September and October 2026.

When we ran that test on 12 October, we turned web search on, set the user location to each chain's city and asked one question a patient would type there.

MeasureHealthcare chains
At least one Business Profile sent to the homepage36% (100 of 279)
Location pages without structured data carrying the street address53% (84 of 158)
Median gap between most and least reviewed location6.3x
Chains blocking ChatGPT's or Perplexity's search crawlers2 of 481

The ChatGPT figure rests on 95 answers that change between runs, so read it as direction, not a rate. Crawler access is almost never the problem; wrong profile links and missing markup are common, and both are fixable in weeks. Review gaps are the larger issue: when one location has a sixth of the reviews of its strongest sibling, that market is where a competitor wins the comparison.

Javvo found 43.6% of medical practices holding a Google top-3 position were never cited by any AI engine it tested, and Glace found only about 2 of the 5 or 6 practices ChatGPT and Gemini named stayed the same between runs. In Halcy's map of 33,340 ChatGPT citations for medical practices, published with MGMA, your own pages are one of the few sources you control.

Questions healthcare groups ask us

What does medical SEO mean?

Medical SEO is search work for providers whose pages give health information, which Google treats as "Your Money or Your Life" content held to a higher standard. For a healthcare group it means accurate listings per location, provider pages with real credentials, clinically reviewed copy, healthcare structured data, and review replies that never confirm who is a patient.

How much does an SEO service cost for a healthcare group?

Prices on competitor pages range from $750 a month for a solo physician at Intrepy to $10,000 a month on a 12-month contract at uSERP. Percepture's guide puts multi-location brands at $5,000 to $15,000 a month. Ours is a formula: $1,900 a month plus $290 per location on Essential, so 10 locations cost $4,800 a month.

What are the top healthcare SEO agencies in the US?

Check who wrote a ranking of medical SEO companies before trusting it. First Page Sage, SEO Discovery, Siege Media and webserv each place their own agency first in lists that rank for this search. Ask any agency for prices in writing, contract length, who owns the work, per-location reporting, and how its review requests and tracking stay inside HIPAA and FTC rules.

What are the types of SEO services?

Most guides split SEO into on-page, technical, off-page and local work, where each location competes in its own map pack. A healthcare group now needs a fifth: AI answer visibility, where engines name a few providers per city. We price all five as one program because they fail together when listings and pages disagree.

What is the 80/20 rule in SEO?

It is the idea that a small share of pages or efforts brings most results. In healthcare chains it shows up in reviews: in our study the median brand's best-reviewed location had 6.3 times the reviews of its weakest, and 56% of chains had a gap of 5x or more. We start work where that gap is widest.

Can a practice reply to Google reviews without breaking HIPAA?

Yes, if the reply never confirms the reviewer was a patient or mentions treatment. HHS has settled several cases over replies that did, including $23,000 with New Vision Dental in 2022 and $30,000 with Manasa Health Center in 2023. We write reply templates for each location that thank the reviewer and move details to a private channel.
